Forms & Procedures

Step 1:  New patients please print, complete, and bring the Initial Intake Forms to your first appointment.

Step 2 Please complete the nutritional survey at least 24 hours before your scheduled appointment.

Step 3:  

Please bring any recent labs and current prescriptions and supplements to your scheduled appointment.  

Please DO NOT wear any perfumes or heavily scented fragrances.  Our staff and patients are highly sensitive to fragrances.

 

Please read the financial policy below.  There is a 3% processing fee if you use any type of card. Checks and Cash are always preferred.

Payment is due at the time of service.                           
 
Olena Wellness Center accepts the following methods:
 
Cash       
Check      
Debit Card
Credit Cards
Most Health Savings Cards      
PayPal
Any card payment is subject to a 3% processing fee. 
 
Olena Wellness Center does not except insurance or give superbills for submittal.
Some flex health savings credit cards are accepted as long as a superbill is not required.  The 3% professing fee does however still apply.
 
Because Olena Wellness Center is not constrained by insurance regulations, we are free to innovate and implement health strategies that are best for our patients without worrying whether they will be "allowed."  Furthermore, our overhead costs are reduced, allowing us to offer our patients optimal value for their healthcare dollars.  The doctor - patient relationship is direct and personal.  You'll be able to see the difference this makes in your healthcare. 
 
Certain types of claims such as auto accidents with insurance coverage may be exceptions to the above policy and will be accepted on a case by case basis.
